Sheriff Leon Lott announces the completion of the investigation of the May 10th shooting involving a Forest Acres Police Officer.

            

On June 15, 2026, RCSD’s Critical Incident Team finished gathering data and evidence in the shooting of Cody Johnson at Relapse Bar & Grill. This morning, the information and evidence gathered from the investigation was forwarded to the Solicitor’s Office to make a determination of if the shooting was justified or not.

News Release: Shooting Involving Forest Acres Police Officer

The Richland County Sheriff’s Department is investigating a shooting involving a Forest Acres police officer at Relapse Bar & Grill located at 5024 Two Notch Rd.

On May 10, 2026, at approximately 3:30 a.m., a Forest Acres police officer heard gun shots coming from the area of Two Notch Rd. near Relapse Bar & Grill. Another Forest Acres officer began to circulate the 5000 block of Two Notch Rd. when a vehicle driving erratically almost struck the officer’s vehicle. The officer activated his blue lights in an attempt to stop the vehicle. The vehicle continued to drive erratically entering the parking lot. The officer exited his vehicle, giving constant commands for the driver to stop. The driver accelerated, almost striking a security guard and others, at which time the officer discharges his weapon, striking the driver. The driver of the vehicle was pronounced deceased at the scene.


The officer-involved shooting occurred within the jurisdiction of the Richland County Sheriff’s Department. RCSD investigators and Crime Scene Unit responded to process the scene and begin an investigation.


The officer involved is Corporal Daniel Haddad. Corporal Haddad has been employed with the Forest Acres Police Department since July of 2021.
